Prior to scheduling your appointment, please note the following:

• Only one person per appointment time.
• The system will only allow you to schedule one appointment. If you are unable to attend, please cancel your appointment.

Ahead of your appointment:

• Ensure that you have received a confirmation email.
• Print and complete the patient registration form.
• Please report to your scheduled appointment on time with your Delgado Community College ID. You will not be seen without this ID.
